Facelift Surgery: The Foundation of Facial Rejuvenation

About 100 years ago, there was no way to rejuvenate an aging face. Even if you were a princess living in the latter half of the 1800s, there was no choice but to come to terms with your aging face. What is Facelift Surgery? Facelift surgery is a surgical operation that tightens the soft tissues of the face by repositioning them to a more ideal anatomical position, thereby improving signs of aging.

What is the Purpose of Facelift Surgery? The purpose of facelift surgery is to reposition the soft tissues in the cheeks and surrounding areas to a more youthful appearance. Our goal in facelift surgery is to make the person the best version of themselves. Another aim of facelift surgery is to place individuals among the top 10-20% of their age category in terms of appearance within their society.

How is Facelift Surgery Performed? Facelift surgery usually begins with an incision made around the ear. This is known as an incision.

Then, the tissues are lifted like a sheet from the underlying anatomical planes. This step is called dissection.

Subsequently, the tissues are repositioned in a new direction. The direction of tissue movement is called the vector, and the repositioning process is known as re-positioning.

Once the tissues are moved to their new positions, they are fixed to solid structures underneath to maintain their new position. This step is called fixation.

Excess parts of the tissues are trimmed off. This step is known as resection.

Finally, the skin incision is sutured and closed. This step is called suturation.

What are the Advantages of Facelift Surgery? There are three main benefits of facelift surgery:

  1. Enhances Beauty: A well-planned and executed facelift surgery reorganizes the harmony between the soft tissue and the skeletal structure, resulting in a more beautiful facial structure. Wrinkles are reduced, sagging in the jawline and neck (jowling, bulldog) is improved, the details of the underlying bony skeleton are more defined, and folds around the mouth (nasolabial and marionette) are softened.
  2. Provides a Youthful Appearance: Shortly after a facelift surgery, you will appear younger in photos compared to your pre-surgery look. This is referred to as an early benefit.

Who are Suitable Candidates for Facelift Surgery? The best candidates for facelift surgery are individuals between the ages of 45-65 who are in good physical and mental health. Of course, in special cases, facelift surgery can be performed at earlier or later ages. The individuals who need facelift surgery the most and who benefit the most from it are those who fall within the last 10% in terms of facial aging in their age group. In other words, those who appear older than their chronological age are the most suitable candidates for facelift surgery.

What are the Risks of Facelift Surgery? Like any surgical procedure, facelift surgery has its own set of risks. Generally, aesthetic surgery procedures have a very low-risk profile. The risks of facelift surgery can be grouped into three main categories:

  1. Surgical Risks: These include complications such as bleeding, tissue loss, nerve injury, and infection.
  2. Systemic and Anesthesia-Related Risks: These are issues unrelated to the surgery itself but that occur during the surgical process and affect other parts of the body. These complications include deep vein thrombosis, embolism, kidney failure, diabetic crisis, hormonal imbalances, pneumonia, heart attack, and allergic reactions. Systemic complications after facelift surgery are quite rare. These complications can be largely prevented with a careful pre-surgical evaluation process.
  3. Poor Outcomes: These are mostly due to design and application errors during the surgery. Yes, anything made by human hands is prone to mistakes. Sometimes patient factors can also pave the way for poor cosmetic outcomes. Smoking, diabetes, poor wound healing, circulatory disorders, and bleeding disorders can complicate the process and lead to poor outcomes even in the hands of the best surgeon.

How Long Does Facelift Surgery Take? The duration of a facelift surgery varies depending on the surgical technique, the experience and care of the surgeon, and many other factors. It generally lasts between 2-4 hours. In minimally invasive approaches such as mini facelift surgery, the duration is shorter, while in multi-layered deep plane facelifts and neck lifts, the duration can be longer. Additional procedures like eyelid surgery, brow lift, midface lift, laser treatments, chemical peels, and fat injections added to the facelift can also extend the surgery time.

What Methods Are Used in Facelift Surgery? More than 300 surgical techniques have been defined for facelift surgery to date. The most well-known are classic facelift surgeries (skin tightening, subcutaneous lifting), where the skin is lifted and tightened along with the underlying fat tissue. Facelift surgery has many different technical variations such as deep plane facelift, HighSMAS facelift, mini facelift, endoscopic facelift, MACS lift, SMASectomy, SMAS plication, and more.

How Long Do the Effects of Facelift Surgery Last? The rejuvenating and beautifying effects of facelift surgery generally last between 5-10 years, depending on the surgical technique and the individual's biological characteristics. The duration of the effect is shorter in procedures like endoscopic facelift and mini facelift, while it is longer in comprehensive procedures like deep plane facelift and highSMAS facelift. The aging-delaying effect of facelift surgery may last a lifetime. This does not mean that the person will not age, but aging signs will appear more slowly in the areas affected by the surgery.

Will Aging Signs Reappear After Facelift Surgery? Facelift surgery reverses the signs of aging. It slows down the appearance of aging signs after surgery. However, it cannot stop the aging process. Aging signs will inevitably reappear over time. Imagine two sisters. They are identical twins. If we perform facelift surgery on one of these sisters and follow the other without surgery, we can say that the sister who had the surgery will look younger than her non-operated sister at the first year, the tenth year, and the thirtieth year after the surgery. However, both will continue to age. In other words, the results of the surgery will not remain as they were in the third month after the surgery for the rest of your life.

Will There Be a Change in Facial Expression After Facelift Surgery? Yes, there will be a positive change. You will appear more energetic, refreshed, and vibrant. A well-done facelift surgery must look natural. About three months after a well-performed surgery, people who meet you for the first time will not be able to tell that you have had any surgery. However, the expression on the face can change negatively and become artificial after poorly planned and executed surgeries.
